Please read the following and check out my FAQ before submitting a form.

• This form is for custom tattoo requests. If you'd like to book a flash design, please check out the flash page.

• Earliest available appointments are currently 2-4 weeks from now (Saturday appointments are popular, so earliest available will be 3-5 weeks from now)

• Please provide a detailed description of your tattoo idea. While I strive to accept as many booking requests as possible, I also want you to find the best artist for your tattoo! Requests will be reviewed to determine if my style, ability, and personal preference match with your vision.

• Consultations will be over email only and my design process is detailed in my FAQ

• You will receive an automated email confirmation if your form has gone through, even if you do not opt in to receive a copy of your responses

• Please allow up to 3 days for my response. I will reply to all submissions, even if we are not a match. If you do not receive an email back, please check your spam folder and/or verify that your form was submitted. If your idea is approved, I will reply with a link for you to schedule your appointment date/time.

• Please be prepared to pay a deposit of $200 to secure your chosen date and time

• My current rate is $400/hour with a $300 minimum for all custom tattoos. I charge only for time spent actively tattooing.

• Your appointment will be at my private studio at ActivSpace
